Sanborn Place is comprised of 73 living units tailored to the needs of the residents. Our units are broken down as follows:
Level of Support III - (35 Units)
Meets criteria of Managed Care or Group Adult Foster Care Programs, needing personal care and homemaking assistance daily
Level of Support II - (25 Units)
Meets criteria of needing personal care and homemaking assistance on a weekly scheduled program
Level of Support I - (13 Units)
Meets criteria of living without support services or chooses to use support services on an as-needed basis

We are a federally subsidized housing program with available on-site home care programs and services provided by Sanborn Home Care.
Sanborn Home Care
Sanborn Home Care is the sister corporation of Sanborn Place, providing an array of professional home care services to elderly, disabled and/or handicapped individuals. Our service plans are tailored to maintain the individual's specific needs at an affordable cost. Services are available to the residents of Sanborn Place as well as the outlying communities.

Van Shuttle service
We will work with you to develop a specialized plan of care. Sanborn Home Care contracts with Mystic Valley Elder Services and a number of area Visiting Nurse Associations. We also accept private pay.
Our van shuttle service runs three days a week, at a low cost, for our residents and the Reading community. We have seen an increase in use of this service as it assists our population to remain independent. The van will go to local medical appointments, shopping, senior center, etc.

Sanborn Day Services
Sanborn Day is a structured program that includes a variety of health, social and supportive services in a safe and caring environment. Sanborn Day is a medical model that provides a staff of health care professionals, including registered nurses, therapists and a social worker who strive to improve the physical and cognitive abilities of each individual. Our program offers families much needed respite while knowing that loved ones are in a secure and enriching environment.
Our professional staff conducts an assessment of each individual and develops a specialized care plan that offers a variety of services for those suffering from memory impairment or brain injury, including:
- Daily personal care assistance including Bathing, Incontinence Care, Medication Management and Feeding
- Stimulating Structured Activities and Field Trips that provide social opportunities
- Breakfast, Lunch and Light Snacks
- Door-To-Door Transportation for clients within a convenient radius of our facility

Sanborn Day Adult Day Services contracts with Mystic Valley Elder Services, Minuteman, VA and MassHealth, in addition to accepting Long Term Care Insurance. We also accept private pay.
HD Physical Therapy proudly serves the communities of Wakefield, Reading, North Reading, Wilmington, Lynnfield, Saugus, Melrose, Peabody, Stoneham, the North Shore and Essex County Massachusetts. Their contemporary PT clinic was founded in May of 2012, and we are proud to announce our partnership with them as we move into the future with goals to serve our community as best as possible.
